Single Activity Fee

The Single Activity Fee reduced to £1.27

Smoking Cessation Service

The service officially commences on 10th March 2022.

Pharmacies providing this service will be eligible for the following payments: 

  • A set-up fee of £1,000 (which will be paid following registration on MYS to provide the service, having declared the pharmacy is ready to provide the service and relevant staff have undertaken the essential training specified in section 3 of the service specification and passed the e-assessments (where applicable). 
  • For each patient a fee for: 
    • the first consultation of £30; 
    • each interim consultation of £10; and 
    • the last consultation of £40 (the last consultation may be at any point from and including the 4-week review up until the 12-week review). 
  • Only the cost of medicines on the list of products which may be supplied as part of the service which is published in the Drug Tariff will be eligible for reimbursement. The cost of those medicines, if supplied as part of the service, will be reimbursed using the basic price specified in Drug Tariff Part II Clause 8 – Basic Price. No other elements of the Drug Tariff in relation to reimbursement of medicines apply to this service. An allowance at the applicable VAT rate will be paid to cover the VAT incurred when purchasing the supplied medicine.

Transitional Payment changes

From 1 January 2022 changes to the payment levels for the Transitional Payment are being made. These changes aim to reduce the overall level of fees delivered to pharmacy contractors through Transitional Payments within the financial year 2021/22 by £5m.

From 1 January 2022 pharmacy contractors who dispense 101 prescription items or more in any month will receive a monthly Transitional Payment which consists of two separate payments – a dispensing payment and a service payment as set out in the table below.

Transitional Payment changes

From 1 October 2021, pharmacy contractors who dispense 101 prescription items or more in any month will receive a monthly Transitional Payment which consists of two separate payments – a dispensing payment and a service payment as set out in the table below.

For the pharmacy contractors to receive the monthly service payment element of the transitional payment they must have:

  • (i) claimed for at least one New Medicine Service (NMS) in the previous month; and

  • (ii) been registered to provide the Community Pharmacy Consultation Service (CPCS) on the Manage Your Service (MYS) platform for greater than or equal to half of the number of full days in the previous month.

C-19 test kit distribution service

From 4th October service payments will be as follows:

  1. one-off set-up fee of £250 + VAT, which covers set-up costs including creating an SOP for the service and training staff who will be involved in providing the service, and storage costs for the test kits ordered from wholesalers. This payment will be triggered by completion of the registration declaration on the MYS portal.
  2. A service fee of £1.70 + an allowance for VAT per transaction using a collect code; and
  3. A service fee of £1.20 + an allowance for VAT per transaction using the anonymous collection route.

C-19 test kit distribution service

A new Advanced Service – the NHS community pharmacy COVID-19 lateral flow device distribution service – was added to the NHS Community Pharmacy Contractual Framework as of March 29 2021.

The funding for the service comes from outside the pharmacy Global Sum and it is made up of the following elements:

  1. An early sign-up fee of £200 + VAT, for any contractors who sign up to provide the service by 23:59 on 18th April 2021.
  2. one-off set-up fee of £250 + VAT, which covers set-up costs including creating a standard operating procedure for the service and training staff who will be involved in providing the service, and storage costs for the test kits ordered from wholesalers.
  3. service fee of £1.50 + VAT per transaction.

February 2021

Discharge Medicines Service

The Discharge Medicines Service (DMS) becomes a new Essential service within the Community Pharmacy Contractual Framework (CPCF) from 15th February 2021.

setup fee of £400 has been agreed to cover the costs of preparing to provide the service, principally training staff and putting in place a standard operating procedure for the service. Payment details for this fee will be included in the Drug tariff in due course.

Contractors providing the full service will be paid a fee of £35.

Where only part of the service can be provided, in certain circumstances defined in the Drug Tariff, contractors will be paid a partial payment:

Contractors will make a monthly claim for completed DMS provisions via the NHSBSA’s Manage Your Service portal. Summary data on each DMS provided will have to be provided to support the evaluation of the impact of the service, contract monitoring and post-payment verification.

Reimbursement for PPE costs

PSNC and the Department of Health and Social Care (DHSC) have agreed a mechanism by which pharmacy contractors can claim reimbursement for their expenditure on personal protective equipment (PPE) during the COVID-19 pandemic in 2020.

Each pharmacy will be able to make two types of claims, where necessary.

 1) Standard claims – this claim is for a set payment to cover the estimated standard PPE use of pharmacies from 27th February 2020 to 31st October 2020.

Contractors will be able to claim the amount set out in the table below;

2) Exceptional claims – this type of claim can be used to cover any PPE purchased:

  1. between 27th February 2020 and 31st October 2020 in excess of the standard claim value and to be used up until 31st March 2021; and/or
  2. between 1st November 2020 and 31st December 2020* and to be used up until 31st March 2021.

* From 21st September 2020, pharmacies could obtain all their COVID-19 PPE free of charge from the DHSC portal

Claims will be made through the Manage Your Service (MYS) portal before 11.59pm on 12 February 2021. Pharmacies with six or more branches may make a single central claim that must be submitted by email to the NHS BSA via the provided central claim form.

Community Pharmacy Consultation Service

From 1st November 2020, the CPCS was extended across England to include referrals from general practices as well as from NHS 111.

GPs can refer patients to community pharmacies to receive a CPCS consultation for minor illness (unlike NHS 111, GPs cannot refer patients for an urgent supply of a medicine or appliance).

Contractors registered to provide the CPCS will be able to claim a £300 engagement and setup payment if they undertake a range of actions to get ready for rolling out the referral pathway in their area. The requirements are detailed in Annex F of the updated service specification. Claims can be made from 1 November 2020 until 31 March 2021 through the MYS portal.

Pandemic Delivery Service

The Pandemic Delivery Service was restarted from November 5th to December 3rd.

Pharmacies (excluding Distance Selling Pharmacies, who are already contractually obliged to deliver dispensed items to their patients) will be paid an allowance to recognise the work involved in supporting the group of eligible patients with their deliveries. This payment will be made automatically by the NHS Business Services Authority (NHSBSA).

September 2020

Hepatitis C testing service

From 1 September 2020 a fee of £36 will be paid for each Hepatitis C Antibody test completed on an eligible person who injects drug (PWID) by a pharmacy contractor.

Pharmacy contractors will also be reimbursed the cost of the Hepatitis C Antibody test in accordance with the list price as published by the supplier. An allowance at the applicable VAT ratewill also be paid.

The two injections will be worth £200 million on 1st April 2020 (or soon after) and £100 million at the end of April/early May.

Serious Shortage Protocols

Amendments have been made to Parts I, II and IIIA of the Drug Tariff to recognise Serious Shortage Protocols (SSPs). The new wording provides details of the endorsement requirements, reimbursement and remuneration for supplies made in accordance with SSPs.

Reimbursement
Reimbursement is for the medicine or appliance supplied in accordance with an SSP, not the originally prescribed medicine/appliance. Payment is normally calculated on the basis that the exact quantity ordered by the prescriber has been supplied, apart from any supply made against an SSP for an alternative quantity. Additionally, where a supply is made against an SSP, the reimbursement price will account for VAT payment.

Remuneration
Aside of the usual dispensing fees and allowances for any items supplied in accordance with an SSP, pharmacy contractors will receive an additional SSP fee of £5.35 per item.

Community Pharmacy Consultation Service

A new Community Pharmacy Consultation Service (CPCS) launched on 29th October 2019, replacing the NUMSAS and DMIRS pilots.

Funding for the CPCS included an initial supplement to the Transition payment for contractors that registered to provide the service before the below deadlines:

  • Registered by 1st December 2019 – £900 supplement
  • Registered by 15th January 2020 – £600 supplement

When a pharmacy provides the service, they can claim a £14 fee per completed consultation.

In December 2016 the Government imposed funding cuts of £113m for 2016/17, with an additional cut of £95m to follow in 2017/18. At the same time, the existing fee structure was changed as follows:

Professional Dispensing fee changes

The Professional Fee was abolished and replaced by the Single Activity Fee. The value of the Single Activity Fee from December 2016 was £1.13 per item.

Practice Payment changes

Practice Payments were abolished in December 2016.

The value of the Establishment Payment was reduced by 20% compared to previous levels:

Fees and Allowances
Fees and allowances set to deliver £20 million less over the course of the year. In October 2010 PSNC secured agreement from DH to postpone recovery of £20m excess margin until 2011/12. This reduction in fees and allowances is intended to recover this £20m.

Transitional Payment will cease.

Practice Payment now set at 70.3p per item for contractors dispensing over 2,300 items per month. All item thresholds and other Practice Payment levels remain the same.

Additional Fees Amended

Changes made to the additional fees including decreasing the number of extemporaneous dispensing fees and removing the bulk prescription fee. Out of hours additional fee removed from national arrangements. Out of hours arrangements are now commissioned locally by PCTs to meet local needs.
