The aim of the pilot was to facilitate a greater level of engagement between general practice and community pharmacy for the benefit of both professional groups and patients.

The objectives of the pilot were to:

  • demonstrate the value of increased clinical engagement between general practice clinicians and the community pharmacist;
  • support general practice prescribing with the provision of expertise from the community pharmacist;
  • raise awareness with general practices of the clinical contribution the community pharmacist can offer;
  • improve patient outcomes such as:
    • increased patient safety;
    • continuity of care; and
    • management of patients with long term conditions;
  • redefine and improve the potential of the patient-pharmacist relationship; and
  • support the best management and use of NHS resources such as reduction of waste.

This joint working pilot ran between December 2014 and April 2015; however, a bid for funding under the Prime Minister’s Challenge Fund was successful allowing roll out of the scheme across the city from October 2015, commissioned by Sheffield Clinical Commissioning Group.

89910 Walk in my Shoes Project (Lewisham Area) Lambeth, Southwark & Lewisham LPC Completed

Lewisham CCG ran the 'walk in my shoes' project which facilitated inter-professional exchange for GP practice staff and community pharmacy staff. 

This project enabled GPs, other clinical and administrative practice staff to gain practical insight into the working environment of community pharmacy. It also gave the opportunity for pharmacists and their dispensing staff the opportunity to see the working environment of general practice. 

The aim of the project was to promote:

  • more effective working relationships between GP practices and pharmacy staff through shared insight; and 
  • improve the overall ability of healthcare professionals to support patients in the medicines optimisation agenda.

The project is a locally funded (NHS England, North Midlands) GP Pharmacy transformation project designed to maximise patients' health and wellbeing by making efficient use of the skills of both general practitioners and community pharmacists. The programme aims to develop and evaluate new models of care to test whether the quality of patient care can be improved by utilising community pharmacy independent prescribers (CPIP) in both a GP practice and/or a community pharmacy setting.

CPIPs will be part of the wider General Practice team and co-manage patients with long term conditions and urgent care needs. In one pilot site, care for patients in nursing homes is also being tested.

The patient contacts comprise mainly of the following:

  • medication reviews;
  • atrial fibrillation (AF) reviews, hypertension and initiation of anticoagulation in AF clinics;
  • DMARD reviews;
  • nursing home patient reviews;
  • renal patient reviews;
  • secondary care discharges into primary care;
  • medication queries from patients and staff;
  • urgent care requests, where the CPIP is competent to provide care; and
  • minor ailments.
